After contacting the family, the staff at the Aged Care Facility will contact the funeral director if one is nominated. They will then coordinate the transfer of your loved one to the funeral director's facilities.
Death in hospital
Please take the time to make decisions. The hospital can care for your loved one until you are ready for them to be transferred into our care.
You will need to fill in an Authority to Collect form for the transfer to take place. We can arrange for the from to be completed.
Death at home
If the death was expected, please contact their doctor or palliative care team. The death will need to be verified before your loved one can be transferred into our care.
Please contact us on 0416 009 129 and we will then guide you through the next steps.
If the death was unexpected, please call 000 for an ambulance. The police may also be called. Your loved one may need to be taken to the coroner; the police will advise on this and make the arrangements.
We can then assist you with the next steps to arrange transfer from the coroners and assist you with the funeral arrangements.
