After contacting the family, the staff at the Aged Care Facility will initiate the necessary death procedures and reach out to the funeral director if one is nominated. They will then coordinate the transfer of your loved one to the funeral director's facilities, ensuring that all funeral arrangements are handled with care and compassion, while also providing grief support to the family during this difficult time.
Please take the time to make decisions regarding the appropriate death procedures. The hospital can care for your loved one until you are ready for them to be transferred into our care. You will need to fill in an Authority to Collect form for the transfer to take place, and we can assist you in completing this form as you also think about funeral arrangements and the grief support you may need during this difficult time.
If the death was expected, please contact their doctor or palliative care team to initiate the necessary death procedures. The passing will need to be verified before your loved one can be transferred into our care. Please contact us on 0416 009 129, and we will guide you through the next steps, including arranging funeral arrangements. If the death was unexpected, please call 000 for an ambulance. The police may also need to be involved, as your loved one may need to be taken to the coroner; the police will provide guidance on this matter and make the necessary arrangements. We can then assist you with the next steps to arrange the transfer from the coroners and support you with the funeral arrangements, along with grief support during this difficult time.
